Spider veins and varicose veins are two types of vein conditions that affect millions of people, mostly women, around the world.  Both of these conditions involve the appearence of bulging or visible veins in the legs, but they differ in their size, severity, and location.


Spider veins, also known as telangiectasia, are small, thin veins, that appear close to the surface of the skin, usually on the legs and  the face.  They are typically red, blue, or purple in color, and often resemble a spider web or tree branches.  Spider veins are or stinging sensations around the affected area.

Varicose veins on the other hand, are larger and more prominent veins that appear swollen, twisted, and raised above the skin’s surface.  They often develop in the legs and feet, where gravity puts pressure on the veins, causing them to weaken and stretch.  Varicose veins may cause discomfort, pain and swelling, especially after standing or sitting for long periods.  In severe cases, they may lead to skin changes, such as ulceration and discoloration.

Both spider veins and vaicose veins are caused by a varity of factors, including genetics, aging, prequency, hormonal changes, obesity, and prolonged sitting or standing.  Treatment options for those conditions vary depending on the severity and symptoms, but may include lifestyle changes, compression stockings, sclerotherapy, laser therapy, or surgery.

TREATMENT OPTIONS

LASER VS INJECTION

LASER SCLEROTHERAPY

Primarily utilized when the vessels are too small or too many to be feasible for injecting. Location also dictates options for intervention. Commonly used on the legs, hands, feet, ankles, and face.

 

INJECTION SCLEROTHERAPY

Classic sclerotherapy, where a vessel is accessed with a small needle and a sclerosant is introduced to the vein. The lumen of the vein is irritated allowing for involution to occur given time and reabsorption.

Common sites for injection are the hands, feet, ankles, legs, chest, abdomen, face and neck.


POST TREATMENT

The site is wrapped or a pressure dressing is applied if applicable. Maintenance of this pressure dressing is necessary to help prevent recurrence of disease.

With recurrence or more advanced disease the possibility of venous insufficiency/high venous pressure may be promoting the veins to stay open. An ultrasound can help discern if you have venous insufficiency.

Recurrence rates after injection approaches 20%. To prevent recurrence we recommend:

Avoiding heat exposure of the treatment sites.

Limit use of oral anticoagulants to prevent blood from opening treatment areas (ibuprofen, motrin, aspirin, aleve etc)

Postoperative follow up in 2 weeks for reassessment possibly drain treated veins to help prevent staining of the skin.

Total time from injection to resolution of the treated sites can be 6-8 weeks.
